Preparing to Retire

​You should submit your Form 6000 at least one month prior to your effective retirement date. Please note that you cannot file your Form 6000 more than 6 months prior to termination of employment.

Required Documents

Please write your Member ID on each copy you send to us.
  • Member's Birth Certificate 
  • Beneficiary's Birth Certificate - If you name a person as beneficiary of your retirement account, we need a copy of their birth certificate too.

Additional forms to complete

Once we process your Form 6000, if you elect to receive a monthly benefit, complete and return the following:
  • Form 6010, Estimated Retirement Allowance
  • Insurance Application- if you are eligible for insurance benefits and choose to obtain insurance through KRS.

An actuarial or lump sum refund

If you elect to receive an actuarial or lump sum refund complete and return the following
We require additional verification from your employer before we can process a refund which may delay your check.  Retirement refunds are paid through a different timeline than the monthly checks and cannot be issued prior to 45 days after termination.
All required forms and documentation must be filed with our office by the last day of the month prior to your effective retirement date.
