Within 5 years of retirement

  • Attend a Pre-Retirement Education Program  and learn more about retirement. Most of our PREP session are already full, click here to view the schedule.
  • Utilize our online calculator through self service to calculate an estimate of retirement benefits, so you know your post-retirement income.

45-60 Days Before Your Retirement Date

What type of documents are required:

  • Member's date of birth verification, i.e. driver's license, birth certificate or passport.
  • Beneficiary's date of birth verification, i.e. driver's license, birth certificate or passport.

Note: If the last name on the date of birth verification is different than that on the Form 6000 additional documentation will be required to verify name change.

  • Return the Form 6000 to KRS via mail or fax.

Additional Forms to Complete:

AFTER KRS receives and processes the Form 6000, KRS will mail you the Form 6010, Estimated Retirement Allowance.

 Choose Your Payment Option

A member who is retiring should carefully review his payment options because no changes can be made to the payment option or the designated beneficiary of the retirement account on or after the first day of the month in which a member receives his first retirement allowance.

Select one payment option, sign and date. Your signature must be witnessed by either your spouse or a witness.

Return Form 6010 by the end of the month prior to your retirement date in order for the first retirement payment to be deposited or mailed on the 14th of the first month of retirement.

Health Insurance:

Please complete Form 6200, Insurance Application, if you are eligible for insurance benefits. Note: If you choose not to participate in the coverage, you will need to complete the waiver section on the Form 6200; otherwise, you will be enrolled automatically into a default plan. 

Return Form 6200 by the end of the month prior to your retirement date in order for your health insurance to be effective the first day of retirement.  If you miss this deadline, you will be enrolled automatically into a default plan.
